I, parent or legal guardian of the above names player, hereby give my approval for him/her to participate in any and all activities, including the use of the players name and likeness on the BYB website, throughout this basketball season. I assume all risks and hazards incidental to such participation including transportation to and from all activities. I also waive, release, absolve, indemnify and agree to hold harmless the organizers, supervisors, participants, and person(s) transporting my child to and/or from activities from any claim arising out of any injury to my child.
